New Information
- Revised All Kids Application – The 2378MC and 2378KCC All Kids applications have been revised to reflect the federal citizenship requirement for U.S. citizens, and an Illinois employer disclosure law that was signed into effect in 2006.
The All Kids application revised in April 2006 will still be accepted for processing.
The revised All Kids application, 2378MC, which contains the AKAA Checklist and Technical Assistance Payment Request, is available for download at www.allkids.com/akaa/app.html. The new application is not yet available for shipping. Shipping availability will be addressed in the next Alert.
- Renewal process for All Kids Premium Level 2-8 enrollees – All Kids Premium Levels 2-8 cases do not have passive renewal and these families must send back the renewal paperwork and income verifications in order to continue coverage.
Some children enrolled in Premium Levels 2-8 will only be able to continue receiving All Kids if they do not have affordable health insurance available to them.
Individuals subject to the health insurance affordability test include newborns and children who were enrolled in All Kids Premium Levels 2-8 as a result of a parent losing health insurance because they lost employment.
The All Kids Central Unit will send out form 643AK, Health Insurance Information for All Kids, to these families along with the renewal notice.
Families that are found to have access to affordable insurance will be sent a notice advising them that their All Kids enrollment will end, giving the family a minimum of 30 days notice to enroll in the employer’s plan. If families report that they are unable to enroll their child, All Kids will ask for written proof from the employer and review the child’s eligibility for All Kids.
- Premium Levels 2-8 Insurance Cards – Starting June 2007, the All Kids cards for Premium Levels 2-8 now include the specific premium level in which the children are enrolled.

2007 All Kids Premium Levels 2-8 Income Guidelines – An updated chart with income guidelines for All Kids Premium Levels 2-8 can be found at the All Kids Web site.
- All Kids, FamilyCare and Moms & Babies Member Handbooks – Member handbooks have been mailed to each enrolled All Kids, FamilyCare and Moms & Babies household. These handbooks include information about how to use the insurance card, how to find a provider and other important information.
- All Kids Customer Site – Enrolled All Kids, FamilyCare and Moms & Babies clients can visit Information for Customers to find important information about covered services and how to use their insurance card. Families can also download a copy of the member handbook (pdf). Families with Share, Premium or Rebate cases may also submit a change of address through this site.
- Illinois Healthy Women Expansion  - Illinois Healthy Women (IHW) is a family planning program for Illinois women age 19 – 44 with income at or below 200% FPL.
Starting May 2007, IHW was expanded to allow women to file an application for this coverage. Previously, the only way to get IHW was to lose eligibility for All Kids, FamilyCare or Moms & Babies.
Clients may refer to IHW as the "pink card." IHW provides services limited to family planning, including birth control and reproductive health services delivered during a family planning visit.

- Illinois Health Connect Update – Letters and enrollment packets have been mailed to persons eligible for Illinois Health Connect in Central and Southern Illinois. Families are encouraged to choose a PCP by responding to the mailing, calling 1-877-912-1999 (TTY: 1-866-565-8576) or visiting www.illinoishealthconnect.com.
Proof of Identity and Citizenship for U.S. Citizens
The Departments of Human Services (DHS) and Healthcare and Family Services (HFS) continue to notify families that receive medical benefits and include persons that declare as U.S. citizens that they must provide original or certified copies of documents that establish their citizenship and identity.
HFS is asking AKAAs for assistance in obtaining copies of citizenship and identity documents. In addition to collecting income documentation from families, AKAAs should also ask for the family to provide the original documents listed on the Form HFS 3859D (pdf) that establishes citizenship and identity.
If birth certificates are not available, the applicant can complete a Form HFS 3859B Statement of Good Faith Effort. If identity documents for children under age 16 are not available, the applicant can complete and sign a Form 3859 (pdf) Statement of Identity. HFS or DHS staff will provide these forms to the client. The client will submit the Good Faith Effort form to the DHS Verifications Unit, if needed.
Only U.S. citizens requesting benefits from HFS need to provide these documents.
Instructions for collecting citizenship and identity documentation:
AKAAs should photocopy the original document(s) and include the photocopy with the completed application. AKAAs should certify that they viewed an original or certified copy by signing and dating the copy. An example is below. AKAAs should use the AKAA Header Sheet (pdf) to simplify documentation. Place the documents on the copy machine; place the header sheet over the documents so the certification appears at the top of the photocopied page.

The original document(s) should be returned to the family.
Please note that HFS and DHS are currently not taking negative action against U.S. citizens who do not provide citizenship/identity documents at the initial point of application or at renewal. However, this is subject to change for parents and caretaker relatives at anytime. Children will not be denied or cancelled but we should continue to secure their documents.
Additionally, AKAAs will not be denied the $50 Technical Assistance Payment (TAP) if the citizenship or identity documentation is not provided with the application.

Trainings for AKAAs located South of I-80 are held at various locations around the state each month. If you are unable to attend any of the training sessions and would like to schedule training at your facility, please call 217-557-9315.
Trainings for AKAAs located North of I-80 are held at the HFS building at 401 South Clinton in Chicago.
If you are unable to attend any of the training sessions and would like to schedule training at your facility, please call All Kids liaison staff at the Chicago office. Groups of six or more participants are needed to conduct training on site.
We have added a new refresher training session. It is recommended for individuals who have already attended the regular All Kids/FamilyCare training and have at least six months of experience completing the 2378MC application. The participants will have an opportunity to measure their knowledge of the All Kids/FamilyCare program. This interactive session requires a high level of involvement from participants in the discussion of policy and procedures, as well as specific case scenarios.
