First, find your family size in the column “Family Size.” Be sure to count yourself.
Look only at your family size row. Read across that row to the box where your family’s total monthly gross income falls.
The box will be in the column of the All Kids plan that matches your income.
Read down that column to the cost box below. The cost box shows the Monthly Premium per child, along with the maximum Monthly Premium for your family, and the Maximum Co-Payments per child, per year.
